Please bring your health card to every visit at Grandview Medical Centre. Present it when checking in at the registration desk or using the touch-screen kiosk located beside the appointment desk.

Missing or Invalid Health Cards
If you do not have your health card or if it is invalid (due to expiry, a change in version code, or a name change), you will be charged for your visit at regular OHIP rates before being seen. This fee is fully refundable once you present a valid health card within six months of your visit.

It is your responsibility to keep your health card valid and up-to-date to ensure uninterrupted access to healthcare and to help prevent fraud.
Lost, Stolen, or Damaged Health Cards
- Health Card – change phone number along with verbiage to ….. You should call ServiceOntario INFOline at 1-800-267-8097 or visit a ServiceOntario centre to report a lost or stolen health card. You can also find information about replacing a lost, stolen, or damaged health card on the Ontario.ca website.

Changes to Your Health Card
If your name or address has changed, you must update your information by visiting or contacting your local ServiceOntario office.
