Memory Clinic
The Grandview Memory Clinic is a free service for our patients experiencing memory concerns, those with a family history of dementia, or patients previously seen and now followed by their family physician.
Our Team
Grandview Medical Centre has two Memory Clinic teams, each consisting of a doctor, nurse, pharmacist, and a shared social worker. Memory clinics are held once or twice a month.
How to Access the Clinic
Referrals to the Memory Clinic must be made by your family physician. Patients cannot self-refer. Referrals are reviewed, and appointments are scheduled based on urgency.
What to Expect During Your Appointment
Appointments last 2–3 hours.
Patients must bring a family member or someone who knows them well.
Please bring all medications in their original packaging, as well as any glasses or hearing aids.
During the assessment, our team will:
Take a comprehensive medical and personal history
Conduct cognitive testing, including an evaluation of driving ability (note: this does not include a road test)
If a memory-related diagnosis is identified, follow-up appointments will be scheduled at the clinic.
Additional Information
No additional blood work is usually required for follow-up visits unless specifically recommended.
